Medicaid Service Coordination

Medicaid Service Coordination helps people with developmental disabilities create a plan that enables them to lead a productive, meaningful life.

Directed by the Office for People with Developmental Disabilities (OPWDD) and funded by Medicaid, this program assists people with severe developmental disabilities in creating an Individualized Service Plan (ISP) that enables them to actively participate in the community and lead a productive, meaningful life. A Service Coordinator works in partnership with the individual and family to develop, implement and maintain the person’s life plan that describes who the person is, his/her strengths, capacities, needs and valued outcomes, and secure the supports and services needed to achieve and maintain these goals. Goals are set for all areas of life, but especially those that affect home, work, friends, finance and leisure.

This is done through individualized service planning meetings and circles of support.  Further, the Service Coordinator serves as an advocate, protecting and ensuring that the person’s living environment is safe. This includes intervening when a person feels his/her rights are being denied. The Service Coordinator keeps accurate and current records on activities and services provided to the person and meets at least once a month face-to-face.

Requirements:
Consumers must be enrolled in Medicaid and be determined to be eligible for OPWDD services. This includes documented evidence of mental retardation or a developmental disability.
